01Health coverage

National Tier 1 PPO health coverage for small businesses.

Switch out of expensive HMO-style limited plans and into a National Tier 1 PPO path that removes referral friction for specialist care and puts your health ahead of HMO bureaucracy.

Three plan tiers. One national PPO network. The deductible changes by tier, but the structure stays the same: broad access, no specialist referral maze, and a real large-group path for a small business owner.

For monthly pricing by tier and household type, see the pricing page.

02Who this plan is built for

You join the group to get the group's rate.

It fits a profitable solo owner or a team of 1 to 25 who pays real money for coverage now and wants the kind of group plan a large company offers its staff. You do not build a plan, and you do not change how you run your business.

For pricing by tier and coverage level, go to the pricing page.

What you get

  • Health

A National Tier 1 PPO, offered in three tiers.

  • Payroll and compliance

The operating structure that makes the group plan possible.

  • HR support and workers' comp

The back office most small businesses do not get on their own.

  • Optional extras later

Additional benefits can be defined during discovery if the math holds.

03How you get these plans

You join a large group that already runs them.

You join a PEO co-employment structure: a large, established group that already runs these plans. For plan pricing, you become part of that group. You keep running your business exactly as you do now. Our PEO partner is ESAC-accredited and a NAPEO member, the marks that signal a vetted operator. We keep the partner's name off public pages until you are introduced directly.

The underwriting carrier and the plan documents are shown above. As we add partners, the documents shown here reflect what each one offers.

04Additional benefits

The PEO can offer more than we show on this page.

We keep the public page focused on the main math: the health plan and the operating structure around it. If the number holds, discovery is where you define the rest of the package.

That can include dental, vision, retirement, and other optional benefits, depending on what you want to build for yourself or your team.

What you can explore on discovery

  • Dental and vision

Optional add-ons if you want a fuller employee package.

  • Retirement options

Plan design and fit for an owner-only business or a team.

  • Supplemental benefits

Life, disability, and other extras if they matter to your setup.

  • Contribution choices

Who goes on what, when benefits start, and how you want the package structured.

05Common questions

Answers, before you ask.

Is this real group coverage?
Yes. It is a National Tier 1 PPO, the same kind of group plan large employers offer. A separate, licensed provider runs the plan, and USA OPS connects you to it.
Does the monthly price change with my age?
No. These are group rates, set across the whole group. Your number shows your exact cost, including any dependents.
Can I see the plan documents before I commit?
Yes. The Summary of Benefits and Coverage for each tier is linked above. Reading them costs nothing and commits you to nothing.
What about an employee I am not putting on the plan?
Every W-2 employee has to run through the PEO's payroll, so a person you are not enrolling in benefits is the $75 compliance-only rate: payroll and compliance, no group plan. The $150 full stack adds the group plan and the rest of the benefits on top.
Who runs the plan and handles claims?
A separate, licensed provider runs the plan and handles claims. USA OPS runs the math and connects you to it.
